In accordance with the Unclaimed Property Act of British Columbia, Central 1 Credit Union ("Central 1") has established the following procedures for making a claim for unclaimed property held by Central.

Instructions for Making a Claim

1. Search Central's unclaimed property database.
2. Print and fill in the requested information on the claim form.
3. Complete the form in order to make a claim for monies being held.
4. Attach a separate sheet if additional space is required.
5. If claiming more than one property, please submit a claim form for each property.
6. When complete, please sign your claim form and fax or mail it with supporting documents to the above address marked Attention: Corporate Information Centre. Questions regarding your claim may be directed to the above telephone/facsimile.
7. If you are not able to print the claim form, the necessary information may be submitted in any other written form.
8. If your claim is denied, you will be advised of Central's appeals procedure.

The personal information requested is used to determine the rightful owner of the unclaimed property and is provided voluntarily by the person making the claim. It is used for the sole purpose of administering claims in accordance with the Unclaimed Property Act.
